Dr. Kenzou Tenma is a skilled neurosurgeon engaged to the hospital director's daughter, poised to climb the ranks of the medical world. But one night, a seemingly minor decision alters his life forever. When instructed to abandon his current patient-an immigrant worker-to operate on a famous performer instead, he complies. Though praised for saving the celebrity, the death of the ignored patient weighs heavily on him. When faced with another similar choice, Tenma refuses to prioritize the town's mayor over a young boy named Johan Liebert. This defiance costs him his reputation and standing. Yet after the mysterious deaths of the director and two other doctors, Tenma is cleared of suspicion and eventually becomes hospital director himself. Nine years later, after saving a criminal's life, Tenma is forced to confront his past-the very monster he once saved. Now he must set out to undo the destruction caused by the person he helped survive.
